Proxy access
Some patients find it helpful for another person to have access to their GP Online account to help them access services online. This can be help with ordering medication or to access to some elements of the care record. This is called 'Proxy Access', in different apps. The 'Proxy' is provided with their own personal log in credentials and the practice sets the proxy's level of access, which may be different to the patient’s access. Care Homes and Pharmacies can also have proxy setting granted.
Parents can also request proxy access for their children who are aged under 11. Children aged 11-15 will need to give their consent for someone else to have access to their records. Please read read the information in our policy regarding other chilren's ages.
To request proxy access, please fill a Proxy Online Access Form and email it to nclicb.tmhp.registrations@nhs.net.
This form is also available at reception.
The 'proxy' will be required to show photo ID before access can be granted.
